Volunteer Application

I have read and understand the statements above. I certify that all the information contained in this application is true and correct and hereby give Operation LBD, Inc. (OLBD) the authority to receive records from any other rescue group with which I have volunteered or currently volunteer. I understand that although OLBD takes reasonable care to screen animals for foster care placement, it makes no guarantees relating to the animals’ health, behavior or actions. I understand that I receive foster care animals at my own risk and can decline to accept any animal for which OLBD has asked me to provide care. I acknowledge that OLBD is not responsible for any property damage or personal injury suffered by me, members of my household, including my own animals, or any third parties during a foster placement, and I assume liability to provide adequate controls to prevent such damage or injury. Unless otherwise arranged, the foster parent is responsible for providing all food, litter, bedding, and toys for the animal while it is in their care at home. The foster parent is responsible for transporting the animals to and from for veterinary appointments, surgery, behavior evaluations, vaccinations, etc. OLBD will pay for all the normal and ordinary veterinary expenses during foster care. I am aware that OLBD is a voluntary group dedicated to assisting Boykin Spaniels and I will not be compensated for my volunteer work/time. In consideration of Operation LBD Inc. hereinafter OLBD’s acceptance of my services as a Volunteer to Rescue or Foster or both, I agree that I, my assignees, heirs, distributes, guardians and legal representatives forever release and shall not make a claim against, sue, or attach the property of OLBD, its officers, directors, volunteers, committee members, employees, agents, heirs, successors and assigns for any actions causes of actions, suits, liabilities, claims, demands, damages or causes of any kind or nature whatsoever, whether presently existing or arising in the future, whether known or unknown, which arise out of or are based upon any fact, matter, claim, or thing involving my services as a volunteer. I agree to indemnify and defend OLBD, its officers, directors, volunteers, committee members, employees, agents, heirs, successors and assigns against and save them harmless from any and all demands, claims, causes of action, or judgments, and any and all expenses (including, without limitation attorney fees) incurred in investigating or resisting the same, for injury to person, loss of life, or damage to property occurring during the term of my services as a Volunteer for Rescue or Foster or both, and until all applicable statutes of limitation expire, arising out of any act, omission, or neglect of OLBD, its officers, directors, volunteers, committee members, employees, agents, heirs, successors and assigns. PRIVACY: As a volunteer of OLBD, you hereby covenant to hold in confidentiality and not to disclose outside of OLBD all confidential information collected by OLBD and entrusted to you for your duties within OLBD. Said confidential information shall include, but not be limited to, the personal information of adoption applicants, volunteers, applicants of any sorts, and information that relates to the organization and is of a sensitive nature such as dates of births, addresses, social security numbers, telephone numbers. This confidentiality covenant shall be binding from here on for all such information received as an OLBD volunteer regardless of whether or not you are a current volunteer.
